Proper Positioning in a Wheelchair

Why it matters

Sitting with good posture is important for comfort, safety, and overall health. Proper positioning helps you breathe better, eat safely, prevent pain, and avoid skin breakdown or injury.

Good upright positioning helps:

  • Prevent pressure sores (skin breakdown)
  • Reduce pain and stiffness
  • Improve breathing and endurance
  • Support safe swallowing during meals
  • Promote better posture and muscle use
  • Decrease risk of falls or sliding out of the chair

What Good Upright Positioning Looks Like

Head & Neck

  • Head is upright and centered (not tilted or leaning)
  • Chin is level (not tipped too far up or down)

Shoulders & Upper Body

  • Shoulders are relaxed and even (not shrugged or slumped)
  • Upper body is upright, not leaning to one side
  • Arms are supported on armrests

Hips & Pelvis

  • Hips are all the way back in the chair
  • Weight is evenly distributed (not leaning to one side)
  • Pelvis is neutral (not slouched or overly arched)

Knees & Legs

  • Knees are level with or slightly lower than hips
  • Knees point forward, not falling inward or outward
  • Thighs are fully supported by the seat

Feet & Ankles

  • Feet are flat on footrests or floor
  • Ankles are in a neutral position (not hanging or twisted)
  • Avoid letting feet dangle

Common Problems to Watch For

  • Sliding forward in the chair
  • Leaning to one side
  • Slouching or rounded back
  • Head tilted or drooping forward
  • Feet not supported

Tips for Better Positioning

  • Make sure the wheelchair fits properly. You may require a custom wheelchair for positioning.
  • Reposition every 1–2 hours if possible
  • Use pillows or supports to keep alignment
  • Check posture after transfers and meals
  • Encourage short periods of upright sitting throughout the day

Talk to your healthcare provider if you notice

  • Redness or skin breakdown
  • Increased pain or discomfort
  • Difficulty staying upright
  • Leaning that cannot be corrected easily
